To request sampling on site (in-home blood sampling) please note the following:

The Payvand Clinical & Specialty laboratory has made every effort to provide on-site sampling service to the laboratory clients. Therefore, he will be with you and with you in all cases.

  1. The in-home sampling service is active on the Laboratory’s working days and you need to register your application 48 hours in advance.
  2. In-home sampling service is available for people over 15 years old.
  3. Depending on the type of test desired, the person applying for sampling must meet the requirements prior to the test. General conditions are fasting, convenient sampling time, and so on.

  4. In order to get a sample on-site, a suitable location should be introduced so that our staff.
  5. One of the main conditions for using insurance tariff (basic – supplementary) is to have a valid prescription (not distorted and free of any scratches). Mentioning an exact date for sampling and having a valid doctor’s stamp (appropriate to the test, general practitioner or specialist are the conditions of the accepted version. They will contact you to correct the prescription.
  6. The cost of sampling at the home of the requested test(s) is 100 thousand Tomans (100,000t) per sampling (the cost of transportation will be added based on the sampling area). If another person has a sampling request in the same place, 50 thousand Tomans (50,000t) will be added to the initial amount for each person. It is also necessary to complete and submit a separate sampling form for each person on site.

  7. To book an appointment for sampling in-home, please deposit the amount of 100.000 Tomans (equivalent to 1.000.000 Rials) as prepayment to the card number 6037701157507234 in the name of Dr. Behzad Poopak, Keshavarzi Bank, and upload the image of your deposit at the location in the sampling request form.

Note: As the queuing is limited, in case of cancellation of your booking time, your prepayment amount (deposit) will not be refunded.

All people nominated by the laboratory for sampling are laboratory experts.
